The era of so-called 'early anti-aging,' where individuals prepare for aging from a young age, has arrived. However, as the use of botulinum toxin extends beyond wrinkle reduction to include skin texture improvement, facial contouring, and body shaping, concerns about resistance have become unavoidable.

Botulinum toxin resistance is a reduction or complete loss of effectiveness in patients who have previously experienced its effects at least once. While awareness of the safety of aesthetic procedures has improved compared to the past, the sheer number of available products has left many consumers questioning which option is best for preventing resistance. We spoke with Dr. Kyuho Lee, Director of Blanc de Vie Clinic, to learn more about this issue.

- How do you perceive the awareness of botulinum toxin safety in clinical practice?

With the increasing availability of various botulinum toxin products in Korea and improved accessibility to procedures, the number of patients experiencing resistance is on the rise. Meanwhile, the prevalence of online information emphasizing low prices has led to a lack of proper awareness among consumers regarding the safety of botulinum toxin. It is now crucial for both medical professionals and patients to recognize the risks of resistance and work together to create a safer treatment environment.

- What causes botulinum toxin resistance?

There are various causes of botulinum toxin resistance. In some cases, genetic factors prevent Botox from being effective from the start, a phenomenon known as primary non-response. 

On the other hand, when the effects gradually diminish or disappear after repeated exposure, it is classified as a secondary non-response. In practice, the dosage and frequency of injections can influence immune responses, and high doses or frequent treatments may increase the likelihood of developing immunogenicity.

The amount of complex proteins included in botulinum toxin also affects the development of resistance. As a result, newer products that purify these complex proteins to reduce the risk of resistance have been introduced. Additionally, individual factors such as immune status, age, and pre-existing immune responses cannot be overlooked. In particular, younger individuals are more prone to antibody formation, making them relatively more vulnerable to developing resistance. Those in the younger demographic considering botulinum toxin treatments should be especially mindful of this factor.

- Why are products without complex proteins considered safer?

Complex proteins and inactive neurotoxins are the primary components that contribute to resistance. In contrast, so-called ‘pure toxins,’ which do not contain complex proteins, are formulated with only the neurotoxin, significantly reducing the risk of resistance. In fact, research has shown that products containing complex proteins have a resistance risk nearly ten times higher than pure toxin products.

A representative pure toxin product is Xeomin. Since Xeomin is less likely to trigger antibody formation, it allows for shorter treatment intervals compared to other products. Depending on the treatment area, procedures can be performed at intervals of approximately 1.5 months while maintaining a low risk of resistance, making it a preferred choice among consumers. In particular, younger patients, who are more likely to receive repeated treatments in the future, are often recommended pure toxin products like Xeomin to minimize the risk of developing resistance over time.

- What symptoms may indicate botulinum toxin resistance?

The clearest indicator is a noticeable decrease in effectiveness. If a patient has previously experienced positive results but finds that repeated treatments no longer produce the same effect or show reduced efficacy, resistance to botulinum toxin may be suspected.

However, it is crucial to accurately determine whether resistance has actually developed, as it is sometimes misunderstood. In particular, patients who receive jaw treatments may feel that the procedure is no longer effective. However, this is not necessarily due to resistance; rather, it occurs because the muscles have already significantly atrophied, preventing further noticeable changes.

"It is also important to understand that the duration for visible effects varies depending on the treatment area. Noticeable results can appear within a day or two for areas like the skin, forehead, and around the eyes. However, it can take at least two weeks for larger muscles to experience the full effect. Considering these factors, the most effective and quickest way to address any concerns is to visit the clinic and consult with the treating physician.

- How can patients who suspect botulinum toxin resistance address the issue?

If a patient develops resistance after receiving treatment at another clinic, I recommend using Xeomin at six-month intervals. In cases where resistance is suspected, it is best to avoid regular botulinum toxin products and opt for Xeomin, designed to minimize the risk of resistance. According to international reports, patients who continued using Xeomin while antibodies were still present eventually regained treatment effectiveness. This suggests that as antibodies against complex proteins decrease, the effects of the treatment can be restored.

- You recently contributed as an author to a literature review paper on the immunogenicity of botulinum toxin type A.

Awareness of resistance has increased, but many aspects remain unclear. When a group of dermatologists in Hong Kong proposed systematically organizing the available information on this topic, I had the opportunity to contribute to the paper.

In our study, we emphasized that botulinum toxin is not only used for aesthetic purposes but also for treating various medical conditions, which makes careful usage even more critical. In particular, patients with central nervous system damage often require higher doses, increasing the risk of resistance. Additionally, children with cerebral palsy exhibit strong immune responses and a higher tendency for antibody formation, which can pose significant clinical challenges. These were key points addressed in the paper.

- Awareness of botulinum toxin resistance does not yet seem to be very high.

More effort is needed moving forward. While it is important for medical professionals to listen to patients’ requests, it is even more crucial to carefully assess what they truly need, considering overall facial balance. Given the broad application of botulinum toxin treatments, healthcare providers must take a professional approach and meticulously evaluate aspects that patients might overlook.

- What can patients do to prevent botulinum toxin resistance?

Patients should be able to accurately share their treatment history with their healthcare provider. With the increasing accessibility of aesthetic procedures, many patients receive treatments from multiple clinics. It is important for them to keep a record of the products they have used in the past and the intervals between treatments and provide this information in detail to their doctor. Ideally, to ensure accurate tracking of their treatment history, patients should consistently visit the same clinic from the beginning.

- These days, many patients receive botulinum toxin treatments alongside fillers. Does combination treatment increase the risk of resistance?

It is difficult to say that combination treatments directly increase the risk of resistance. While each procedure may contribute to resistance individually, the combination of botulinum toxin and fillers itself is not a direct factor in the development of resistance.

However, it is important to be aware of potential side effects associated with combination treatments. In rare cases, inflammatory or allergic reactions may occur, and it can be difficult to determine which procedure caused the issue. Therefore, when performing combination treatments, a careful assessment of the patient's condition is essential, with safety as the top priority. A more cautious and thorough approach is required to minimize risks.

- What advice would you give patients who are hesitant about treatment due to concerns about resistance?

Botulinum toxin has minimal side effects, and resistance is the most common concern, even if they do occur. However, this can be prevented through proper product selection and maintaining appropriate treatment intervals. There are many ways to live a youthful and vibrant life, but botulinum toxin is an excellent option for those who wish to avoid and manage aging with medical support. Above all, it is a simple and safe procedure, making it an excellent choice even for first-time aesthetic treatment patients.
